Referral Acceptance Rate

The percent of final referral decisions (the last decision you make on a referral, if you receive the referral multiple times) that were accepted out of the referrals received. The target is 85% for West Side referrals and 100% for East Side (FIN) referrals. Referrals awaiting a decision, manually or auto-resolved before a decision, or received within the last 3 days are excluded. Statewide, networkwide, and regionwide rates show the average rate for all providers (including you) in the state, network, or region.

On Time First Visit Rate

The percent of accepted referrals that had an attended first visit within 5 days of receiving the referral for on-going referrals or 3 days of being entered into Sprout for 72-hour referrals. Referrals received within the last 9 days are excluded. Statewide, networkwide, and regionwide rates show the average rate for all providers (including you) in the state, network, or region.

Total Miles Driven

Totals are for all visit reports during the period. Averages are at the visit report level and include all legs of transport. Reports without transport are excluded.

Total Visit Reports

Approved visit reports during the time period. Average visit minutes are the average number of supervised visit minutes per visit report. Unsupervised and transport-only visits are excluded.

Attendance Rate

The percent of attended visit reports out of all approved reports. Expected visits with no visit reports are excluded. Statewide, networkwide, and regionwide rates show the average rate for all providers (including you) in the state, network, or region.

Average Time to Referral Decision

The average time from receiving a referral to accepting or rejecting it. For 72-hour referrals, time starts when the referral was entered into Sprout. Referrals with no decision or received within the last 3 days are excluded. Statewide, networkwide, and regionwide shows the average for all providers (including you) in the state, network, or region.

Average Time to Submit Report

Average time after a visit ended until the visit report was submitted for approval. If the visit report does not have an end time, e.g., for a cancelled visit, the visit start time is used instead. The target is 1 day. Statewide, networkwide, and regionwide shows the average for all providers (including you) in the state, network, or region.
